• Pulmonary manifestations of HIV disease are common.
  
• The pattern of disease has changed owing to a combination of factors. Plain radiography coupled with clinical assessment remains the starting point of investigation.
  
• CT is usually not required, except as a problem-solving tool.
  
• Whilst infection remains common, there are numerous non-infectious causes of acute chest symptoms.
