• Clinical assessment is essential before imaging.
  
• Patients with motor or significant sensory loss need immediate surgical intervention without delay for imaging.
  
• There is no evidence of significant difference in clinical outcome for any of the different thrombolysis techniques.
  
• Mechanical thrombectomy may debulk thrombus but adjuvant lysis is often required.
  
• Percutaneous aspiration can be useful to treat distal embolus.
