Endovascular management of dissection of the descending aorta

• Aortic dissection is a significant cause of mortality and morbidity.
  
• Mortality may result from cardiac tamponade, rupture or branch vessel ischaemia.
  
• The most widely accepted classification is the Stanford classification.
  
• Stanford type A dissections are conventionally treated by surgery, type B by medical therapy if uncomplicated or by surgery if complicated.
  
• Surgical morbidity and mortality may be high.
  
• Accurate diagnosis is essential.
  
• Endovascular techniques of stenting, stent-grafting and fenestration offer encouraging alternatives to surgery with much reduced morbidity.
  
• Endovascular techniques are in the early phase of development and evaluation.
